APPOINTMENT / SERVICE DEPOSIT:
Appointment and Service Fees must be secured with a Credit Card Authorization for charges not covered by EAP/Insurance (co-pays, missed/canceled appointments, outstanding balances); or claims denied/rejected by Insurance/third Party Payer. The Credit Card Authorization will only be proceeded if the client fails to appear for a scheduled appointment (No Show), cancels or reschedules an appointment without a 24-hour cancellation notice, or when a claim is denied, rejected, or not covered. EFFECTIVE January 2023

Diagnostic Assessments
Diagnostic Assessment involves the collection of data through the use of tests, interviews, case studies, behavioral observation, and other techniques to measure measure development, personality, attitudes, cognitive, social, or emotional functioning/abilities. Assessment data is also used to diagnosis disorders, classify disabilities, determine psychological aspects of a illness, injury or disability; eligibility determination; and treatment planning.
